Why are we asking you to participate?

Cancer survivors have important needs following their treatment, this study will help identify the issues that survivors face.  Project Milestones is a study looking at the lifelong health among young adult cancer survivors.  By participating in our study, you are helping others, like yourself, by talking about your experience as a survivor, and you help us learn more about living long healthy lives.

Those invited to participate in the study, will have received a letter in the mail with a link and access code to take the survey.  If you are invited, please click the red button above to begin taking the survey.

What are you being asked to do?

You are being asked to complete a short, 30-45 minute survey about your health, attitudes, health care, life experiences, relationships, and opinions.  Some questions may be more personal or sensitive than others, including topics of sexuality, financial status, illicit drug use, and citizen status.  Participation is completely voluntary and questions may be skipped if you do not wish to answer them.  We want to hear from as many people invited as possible to get a better understanding of cancer survivorship among young adults.  All information is kept private and confidential.  All study participants will receive a $30 gift card upon completion of the survey.

What are the risks and benefits in participating?

Some questions we ask are very personal or sensitive, and others are more broad.  Your participation is voluntary and you may skip any questions you do not feel comfortable answering.  While all information is kept private and confidential to the fullest extent possible, the loss of confidentiality is a small but possible risk.  There are no direct benefits to you for participating in our study.  Your participation may help researchers improve care for others like you.

Will your information be kept private?

All records that contain your name, telephone number, or other personal information are kept separate from the survey forms.  All forms are kept in a locked file cabinet and computer files are password protected.  The website we used for the survey is also password secured.  Only study personnel will be able to see your records, and all study personnel are trained to keep your information private and confidential.  They have also signed confidentiality letters.  We may publish the information from the study in journals or present it at meetings.  If we do, we will not use your name.

How can you participate?
